Around the world, depression is a common disorder, yet it is widely misunderstood and stigmatized. While everyone has episodes of sadness, depression is more than temporary emotional low points. It is a deep and complex condition that can affect how a person thinks, feels, and functions daily. The World Health Organization says an estimated 280 million people around the world suffer from depression. That makes it not only a common condition but also one of the leading causes of dysfunction and deep disability.

          What Is Depression?

Major depressive disorder (MDD), also known as depression, is marked by:

           a sustained period of sadness,

a disinterest in previously enjoyed activities,

a tiredness that doesn't seem to improve with rest,

           an inability to focus,

and either an insatiable appetite or a lack of one (American Psychiatric Association, 2022).

To be diagnosed with MDD, a person must show these signs for at least two weeks, and they must not just be personal roadblocks; these signs must also greatly hinder them in the domains of life that are social, professional, or otherwise meaningful.

When a definite life occurrence—such as sorrow or trauma—does not seem to be the cause of depression, it may come upon a person without any clear or apparent reason. This type of depression seems to arise "out of the blue" or develop insidiously over time. It most definitely occurs within persons of certain biological, genetic, or family backgrounds that make them particularly vulnerable to not just depressive episodes but also to other types of mental disorders.

          The Neurobiology Behind Depression

Neuroscience has made great strides in understanding the biology of depression. Recent advancements in knowledge have increased investigations into the very biological underpinnings of depression. We now know that it is not just a behavioral or psychological ailment, but one that also deeply affects the body—specifically the brain. Structure-function abnormalities in the brain, particularly in the amygdala, hippocampus, and prefrontal cortex (Disner et al., 2011), are consistently found in depressed individuals. Neurochemical imbalances in certain critical neurotransmitters like serotonin, dopamine, and norepinephrine also deeply affect our moods (Stahl, 2013).

Some people are more likely than others to develop depression because of certain risk factors. These include:

Statistically, a greater number of women receive a diagnosis of depression. This is probably because men don't report the kind of symptoms that are diagnostic for depression. They don't report them because they aren't in touch with the kind of emotions that would lead them to say, "I'm feeling x, y, and z," as a vulnerable, emotionally expressive woman would say. (Kuehner, 2017)

Although depression is very treatable, the path it takes varies from person to person. The effective treatments for depression include:

Counseling: Cognitive-behavioral therapy (CBT), interpersonal therapy (IPT), and psychodynamic approaches are the most frequently employed counseling strategies. They exhibit a robust track record of effectiveness.

Medications: Patients with mood disorders are often prescribed selective serotonin reuptake inhibitors (SSRIs) or serotonin-norepinephrine reuptake inhibitors (SNRIs) to help regulate their mood.

Interventions in Daily Living: Clinically significant reductions in symptoms of anxiety and depression have been observed in individuals who engage in consistent, vigorous exercise (Schuch et al., 2016). Maintain an exercise regimen with both cardiovascular and strength-training components for at least 30 minutes a day, 4 to 5 days a week.

Support systems: Interaction with friends, family, or peer support groups can yield a profound sense of community and mutual understanding. Hearing someone say, "I understand what you're going through, I've been there" can be a vital lifeline to continue on a tough day.

Prompt responses are critical. Untreated depression can lead to far-reaching consequences, such as self-harm and fatal outcomes. The National Institute of Mental Health (2023) underscores the importance of recognizing early warning symptoms—and taking those seriously enough to get help from mental health professionals.

It is vital to remove the stigma from mental illness if we are to get more people in treatment and on the road to recovery. A person with depression is not weak or morally lacking. They have a bona fide health problem. Our society thinks nothing of a person going to a doctor for a chronic condition like asthma or a life-threatening illness like cancer. Yet, we cannot seem to get over the idea that it is okay to go to a psychiatrist for depression or anxiety. We must educate the public.

Anxiety is a common emotion, yet for millions of people, it transforms into more than just an occasional moment of worry. Instead, it takes on a predominant and persistent life of its own and becomes the kind of overwhelming experience that disrupts daily living. In our speedy, high-stress society, understanding this fundamental emotion is not only our clinical responsibility but also a public health imperative—that is, a priority for the health of the population at large.

          What is Anxiety?

Feeling tense, worried, or on edge? You might be experiencing anxiety. It's an emotional state we all go through that ranges in intensity and duration and is most often experienced in relation to specific events or stressors. According to the American Psychological Association (2023), here is how they define it:

Anxiety is an emotional state characterized by feelings of tension, worried thoughts, and physical changes like increased blood pressure. While anxiety is a natural reaction to stress and can even be beneficial in some situations, anxiety disorders occur when these feelings become chronic, excessive, and difficult to control.

The fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) outlines several anxiety disorders, including generalized anxiety disorder; panic disorder; social anxiety disorder; and specific phobias (American Psychiatric Association, 2013). These disorders present some unique symptoms but often share common threads such as:

          Root Causes of Anxiety

A complex interaction of biological, psychological, and environmental factors can result in anxiety. Genetics are significant—they play a big role (when it comes to who gets diagnosed with it) and family studies clearly show that anxiety disorders can run in families (Hettema, Neale, & Kendler, 2001). And then there are certain areas of the brain that are linked to anxiety. Neurobiologically, the heightened activity in the amygdala (the brain's fear center) has been linked to anxiety responses (Shin & Liberzon, 2010).

Another thing to consider is that a lot of our factors are psychosocial as well. When we consider psychosocial factors, we pay attention to things like trauma, adversity, and stress that children have experienced early in life. These things can make a person much more likely to end up with anxiety. We also know that perfectionism, which many of our A students have, also increases the chances of being anxious. But no one's had any control over the kinds of diets or activities that have been promoted, which are certainly far from perfect and are very stressful.

Thankfully, anxiety can be treated very effectively. The most successful treatments are generally forms of psychotherapy combined with innovations in the patient's lifestyle and, when necessary, medications. The cognitive behavior modifications performed with patients during therapy have made CBT the gold standard for the non-drug treatment of anxiety disorders.

Practices that are based on mindfulness, like meditation and deep breathing exercises, have shown favorable results in the calming of the nervous system. These practices have also shown an increase in the awareness of the present moment. (Khoury et al., 2013)

Other tools that can help diminish anxiety symptoms over time include:

           Regular sleep

           Social connection

           Proper nutrition

Some people may be prescribed medications, such as selective serotonin reuptake inhibitors (SSRIs), to manage the biochemical imbalances that lead them to experience anxiety. These medications should always be taken under the supervision of a licensed healthcare provider.

Coexisting with apprehension can be disconcerting, but it is crucial to keep in mind that assistance is at hand and that recovery is within the realm of possibility. Routine or irregular panic attacks can leave one feeling as though they have lost control or that their life is in danger when, in actuality, the body is just overreacting to a threat that isn't really there. Because this overreaction often leaves one feeling exhausted, it's important to schedule practices for self-care in a way that makes them feel manageable and to learn to ask for help when needed.

If you or someone you are acquainted with is going through a tough time with anxiety, you might think about getting in touch with a mental health expert. Making the call for help can be the first but also the toughest step. Yet, most who take it find that they are met with understanding and compassion, and that the experts they encounter really do want to help. And though it might be hard (or impossible) to see at the moment, many who go for help find that they come out on the other side stronger and more capable. It can be life-changing.
